For better or worse, decisions made about pharmaceuticals have a significant impact on lives around the world. Just how big is the pharmaceutical industry in the U.S. and worldwide and what is its impact on the economy? How do pharmaceutical companies decide what drugs to pursue or abandon? Where and how are drug trials conducted and published (or not)? Just what percent of revenues is actually spent on R&D, by whom, and how does that compare to expenditures on marketing drugs to consumers and doctors? How is medication priced and does the answer vary based on who the purchaser is or where the purchaser lives? What are "essential medicines" and how does global tiered pricing affect consumers in different countries? What about off-label marketing and use, deaths caused by medication, and prescription drug abuse (such as opioids)? The World Health Organization says that there is "an inherent conflict of interest between the legitimate business goals of [pharmaceutical] manufacturers and the social, medical and economic needs of providers and the public to select and use drugs."
