America's infatuation with the intuitively appealing concept of "population health" risks blindness to its Achilles' heel -- it doesn't work. The country is at a crossroads. Health reform promises to pay for expanded insurance coverage and incremental spending with savings promised by promoting wellness, preventing or postponing illness, and using primary care medical homes to avoid expensive episodes of care. Unfortunately, neither economics nor biology support the traditional view of population health. The good news is that there are answers to the problem of unsustainable health care spending...the bad news is that we do not have another ten years to devote to old thinking.
